[ARCHIVED] Adults with Intellectual / Developmental Disabilities

The City of Leesville is conducting a survey to discover the number of Vernon Parish residents who might benefit from having a local facility that would provide assistance to adult citizens with intellectual and developmental disabilities.

The proposed facility might offer lifestyle enrichment opportunities, as well as job training and placement services and other age-appropriate activities that would enable a sense of achievement and as much independence as possible.

If you would be interested in this exciting opportunity, please complete this form. Your information will never be shared. At this point, we are only interested in numbers so that we can show the need for such a facility. However, if more information is required, we may need to contact you.

*In order to receive the benefits of the proposed facility, you must be certified with a Waiver K from Medicaid. This process may take up to 6 months, so should be started as soon as possible. Please contact the Region 6 Office for Citizens with Developmental Disabilities: Central Louisiana Human Service District at 318-484-2310.
